This form outlines the nature of the coaching services Olive Joy Ministries provides and seeks your consent to participate in coaching and holistic counseling. It also clarifies the client's and counselor's rights and responsibilities.

Confidentiality Agreement

This agreement highlights our commitment to maintaining the confidentiality of your personal information and the content discussed during sessions. It explains the limited circumstances in which confidentiality may be breached, such as instances of imminent harm or legal requirements.

Release of Information Form

This form allows you to grant permission for Olive Joy Ministries to share relevant information with other professionals or organizations involved in your development journey.

Educational History and Goals Form

Informed Consent for Minors (if applicable)

If coaching and/or holistic counseling services are provided to a minor (under 18 years old), a parent or legal guardian must complete this form, providing informed consent for their child's participation in coaching and/or educational counseling.
